Job Summary

As a Transplant Coordinator RN, you will utilize a nursing model to deliver high-quality patient care. You will assess patient needs and create personalized care plans, ensuring smooth transitions through the healthcare system and preventing complications or delays.

Key Responsibilities
  • Assess transplant eligibility for patients through interviews and family discussions
  • Coordinate tests for transplant candidates, identify any issues in results, and arrange necessary care
  • Manage recipient's post-transplant care, including arranging admissions, lab tests, and clinic visits
  • Coordinate with referring and primary care physicians, understanding test results and identifying irregularities
  • Set up needed care as directed and teach candidates, families, and caregivers about evaluation, organ allocation, waiting, post-operative, immunosuppression effects, complications, and psychological impact of transplant
  • Use insurance knowledge to help with post-transplant prescriptions and assist patients with coverage issues
  • Maintain long-term prescriptions and comply with various regulatory policies, maintaining records on the transplant waitlist through different online programs
  • Coordinate recipient transplants, using the donor website and communicating with potential recipients, assessing their health status, and notifying hospital departments
Requirements
  • Associate's degree level
  • Two years of professional experience
  • RN License
  • Basic Life Support skills required or obtain BLS certification within 30 days if not already held
