Job Summary

This position is responsible for scanning and indexing historical and current patient information into our electronic health record system, ensuring accurate and efficient documentation.

Responsibilities
  • Assist physicians and mid-level providers with chart completion and accuracy
  • Pull and file paper medical records as needed, maintaining confidentiality and security
  • Review documents received to determine if they require scanning and indexing
  • Prepare documents for scanning according to policy and procedure, ensuring accuracy and completeness
  • Index medical record documents, verifying patient identification and accuracy of scanning level
  • Suspend documents without identifiable document type or patient issues for quality review
  • Review documents for proper frame, resolution, orientation, and adjust accordingly or notify staff if rescanning is required
Requirements
  • High School Diploma or GED
  • Clinical experience preferred, with knowledge of basic medical terminology
  • Understanding of chart order and ability to categorize documents
  • Ability to work with scanning equipment and review records for accuracy
