Position Title: Interim Housing Day Shift Case Manager
Location: Santa Barbara, CA
Compensation: $23.24 to $28.28 per hour
Employment Type: Full Time

ABOUT PATH
Founded in 1984, PATH (People Assisting The Homeless) has been at the forefront of innovative strategies to support individuals experiencing homelessness. We provide a comprehensive range of services throughout California, connecting clients with essential resources including homelessness prevention, outreach, employment assistance, individualized case management, supportive services, interim housing, and permanent supportive housing.

ROLE DESCRIPTION
The primary responsibility of the Case Manager is to support clients in overcoming homelessness through a comprehensive, client-centered approach. This role involves assisting clients at various stages of homelessness, including street outreach, interim housing, transitional housing, and permanent housing solutions. The Case Manager will facilitate access to necessary services, including healthcare, social services, and support systems, while navigating clients through a coordinated entry system.

  • Conduct comprehensive intake assessments and collaborate with clients to create Individualized Service Plans (ISP) that focus on housing objectives, income enhancement, and other personal goals, including health, mental wellness, substance use, financial stability, vocational training, and social support.
  • Provide referrals and link clients to resources that assist in achieving their ISP goals.
  • Administer program-specific assessments as required.
  • Evaluate client strengths and challenges in relation to short-term and long-term objectives, conducting a 90-day review post-intake.
  • Maintain accurate documentation in compliance with program and organizational policies, ensuring timely case file updates.
  • Uphold client confidentiality in accordance with HIPAA and relevant guidelines.
  • Foster therapeutic and professional relationships with clients and community partners to enhance engagement and support housing goals.

PREFERRED EXPERIENCE
Two years of experience in healthcare, case management, homeless services, or related fields is preferred.

MINIMUM REQUIREMENTS
All levels of education and experience are considered.

MANDATORY CRITERIA
All candidates must:

  • Provide employment eligibility verification.
  • Obtain CPR/First Aid certification.
  • Complete a Tuberculosis Test, Background Screening, and Drug Test as a condition of employment.

DRIVING REQUIREMENTS
Driving is an essential function of this role. Candidates must possess:

  • Reliable transportation.
  • A valid driver's license.
  • Proof of insurance and ownership for vehicles used for work duties.
  • Eligibility for PATH's insurance coverage.

EMPLOYMENT ELIGIBILITY

WORK LOCATION
All work must be performed within California, as per state regulations.
