Position Title: Interim Housing Day Shift Case Manager
Salary Range: $23.24 to $28.28 per hour

Founded in 1984, PATH (People Assisting The Homeless) has been at the forefront of innovative and effective strategies to support individuals facing homelessness. Our services span across California, connecting clients with a comprehensive range of resources including homelessness prevention, outreach, employment support, case management, and both interim and permanent housing solutions.

ROLE DESCRIPTION
The primary responsibility of the Case Manager is to empower clients to overcome homelessness through a comprehensive and personalized approach. This role involves supporting clients through various stages of homelessness, including street outreach, interim housing, transitional housing, and securing permanent residences. The Case Manager will facilitate access to essential services, including healthcare, social services, and community support systems.

  • Conduct thorough intake assessments and collaborate with clients to create Individualized Service Plans (ISP) that focus on housing objectives, income stability, and other personal aspirations, including health and social support needs.
  • Provide referrals and connect clients with resources that assist in achieving their ISP goals.
  • Perform program-specific assessments to evaluate client needs.
  • Assess client strengths and challenges in relation to their short-term and long-term goals, conducting follow-up reviews.
  • Maintain accurate and timely documentation as per program and organizational policies, ensuring confidentiality in accordance with HIPAA guidelines.
  • Foster professional relationships with clients and community partners to enhance engagement and support for housing goals.

QUALIFICATIONS
We are seeking candidates who possess:

  • Strong analytical skills and attention to detail, with the ability to interpret data and inform decision-making.
  • Collaboration skills to work effectively with colleagues and stakeholders.
  • Excellent communication abilities, both verbal and written, to convey information clearly.

PREFERRED EXPERIENCE
Two years of experience in healthcare, case management, or related fields working with vulnerable populations is preferred.

MINIMUM REQUIREMENTS
All levels of education and experience are encouraged to apply.

MANDATORY CRITERIA
All candidates must meet the following requirements:

  • Verification of employment eligibility.
  • Willingness to obtain CPR/First Aid certification.
  • Successful completion of a Tuberculosis Test, Background Screening, and Drug Test as a condition of employment.

DRIVING REQUIREMENTS
Driving is a critical function of this role. Candidates must have:

  • Reliable transportation.
  • A valid driver's license.
  • Proof of insurance and ownership for vehicles used for work-related duties.
  • The ability to qualify for the organization's insurance coverage.
