Position Title: Housing Support Case Manager
Location: Santa Barbara, CA
Compensation: $23.24 to $28.28 per hour
Employment Type: Full Time
Classification: Non-Exempt

We are seeking dedicated individuals who are passionate about facilitating positive changes in the lives of others to join our Interim Housing Services team as a Housing Support Case Manager.

We provide a wide range of services throughout the state, connecting clients to essential resources for homelessness prevention, outreach, employment support, case management, and housing solutions.

ROLE RESPONSIBILITIES
The primary responsibility of the Case Manager is to empower clients to overcome homelessness through a comprehensive and supportive approach. This role involves guiding clients through various stages of housing instability, including street outreach, interim housing, transitional housing, and permanent housing solutions. The Case Manager will assist clients in accessing vital services related to healthcare, social support, and navigating the coordinated entry system.

  • Conduct comprehensive intake assessments and collaborate with clients to create tailored Individualized Service Plans (ISP) that focus on housing objectives, income enhancement, and other personal aspirations, including health, mental wellness, and vocational support.
  • Provide referrals and connections to resources that facilitate the achievement of ISP goals.
  • Administer program-specific assessments as required.
  • Evaluate client strengths and challenges in relation to their goals, conducting regular reviews to assess progress.
  • Maintain accurate documentation in compliance with program standards and organizational policies, ensuring timely updates to client files.
  • Uphold confidentiality standards in accordance with HIPAA and relevant guidelines.
  • Foster professional relationships with clients and community partners to enhance engagement and support for housing goals.

QUALIFICATIONS
We are looking for candidates who possess:

  • Strong analytical skills and attention to detail, with the ability to interpret data and identify trends to inform decisions.
  • Collaborative spirit, working effectively with colleagues and stakeholders to achieve common goals.
  • Excellent communication skills, both verbal and written, to convey information clearly and engage diverse audiences.

  • Commitment to ethical standards and professional boundaries in all interactions.

PREFERRED EXPERIENCE

  • Two (2) years of experience in healthcare, case management, homeless services, or related fields.

MINIMUM REQUIREMENTS

  • All levels of education and experience are welcome to apply.

MANDATORY CRITERIA
All candidates must meet the following requirements:

  • Provide proof of employment eligibility.
  • Be willing to obtain CPR/First Aid certification.
  • Successfully complete a Tuberculosis Test, Background Screening, and Drug Test as a condition of employment.

DRIVING REQUIREMENTS
Driving is a crucial aspect of this role. Candidates must:

  • Have reliable transportation.
  • Possess a valid driver's license.
  • Provide proof of insurance and ownership for vehicles used for work-related duties.
  • Qualify for the organization's insurance coverage.

EMPLOYMENT ELIGIBILITY

WORK LOCATION
All work must be performed within the state of California, and approval for remote work does not imply permission to work outside the state.
