Salary Range: $23.24 to $28.28 per hour

ABOUT PATH
Founded in 1984, PATH (People Assisting The Homeless) has been at the forefront of innovative solutions to assist individuals facing homelessness. We provide a wide array of services across California, connecting clients to a comprehensive continuum of support, including homelessness prevention, outreach, employment assistance, individualized case management, interim housing, and permanent supportive housing.

ROLE DESCRIPTION
The primary responsibility of the Case Manager is to support clients in overcoming homelessness through a comprehensive, client-centered approach. This role involves assisting clients at various stages of homelessness, including street outreach, interim housing, transitional housing, and securing permanent housing. The Case Manager will help clients access essential services, including healthcare, social services, and support systems, while navigating the coordinated entry system.

  • Conduct thorough intake assessments and collaborate with clients to create Individualized Service Plans (ISP) that address housing objectives, income enhancement, and other personal goals, including health, mental well-being, and social support.
  • Provide referrals and resources to assist clients in achieving their ISP goals.
  • Complete program-specific assessments as required.
  • Evaluate client strengths and challenges in relation to their short-term and long-term goals, conducting regular reviews.
  • Maintain accurate documentation in accordance with program policies, ensuring timely case file updates.
  • Uphold client confidentiality in compliance with HIPAA and relevant guidelines.
  • Foster professional relationships with clients and community partners to enhance engagement and support for housing goals.

QUALIFICATIONS
We seek candidates who possess:

PREFERRED EXPERIENCE
Two years of experience in healthcare, case management, or related fields, particularly with vulnerable populations.

MINIMUM REQUIREMENTS
All levels of education and experience are welcome.

MANDATORY CONDITIONS
All candidates must meet the following:

  • Verification of employment eligibility.
  • Willingness to obtain CPR/First Aid certification.
  • Successful completion of a Tuberculosis Test, Background Screening, and Drug Test as a condition of hire.

DRIVING REQUIREMENTS
Driving is a crucial aspect of this role. Candidates must possess:

  • Reliable transportation.
  • A valid driver's license.
  • Proof of insurance and ownership for personal vehicles used for work.
  • Eligibility for PATH's insurance coverage.
