Crestwood Behavioral Health is seeking a dedicated and compassionate Service Coordinator to join our team. As a key member of our interdisciplinary team, you will play a vital role in supporting individuals in their recovery journey.

Job Responsibilities

  • Assess clients to identify individual problem areas necessitating treatment and complete comprehensive assessments.
  • Coordinate interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team.
  • Implement the overall treatment plan for clients on the unit, working closely with nursing staff to ensure continuity of treatment efforts.

Requirements

  • Prefer a bachelor's degree in psychology, social work, or a field related to mental health.
  • Two (2) years of full-time experience or part-time equivalent in a mental health setting.
