Job Description

We are seeking a highly motivated and compassionate Service Coordinator Mental Health Case Manager to join our team at Crestwood Behavioral Health. As a key member of our interdisciplinary team, you will play a vital role in assessing clients to identify individual problem areas necessitating treatment and completing assessments.

Key Responsibilities:

  • Assess clients to identify individual problem areas necessitating treatment and complete assessments.
  • Coordinate the interdisciplinary team meetings and discharge planning efforts among the multidisciplinary treatment team.
  • Implement the overall treatment plan for the client on the unit, working closely with the nursing staff to ensure continuity of treatment efforts.

Requirements:

  • Prefer a bachelor's degree in psychology, social work, or a field related to mental health.
  • Two (2) years of full-time experience or part-time equivalent in a mental health setting.
