Position Overview:

The role of a Caregiver involves providing essential support to clients in accordance with a detailed care plan established by the agency's Care Manager. This includes both direct personal care and supportive tasks that enhance the client's quality of life.

Key Responsibilities:
  • Assist clients with daily living activities as outlined in their care plan, which may encompass companionship, meal preparation, light housekeeping, transportation, shopping, medication reminders, bathing, dressing, feeding, and mobility assistance.
  • Maintain open communication with the Care Manager regarding any changes in client condition or care needs, as well as any safety concerns.
  • Adhere to company protocols for infection control and ensure proper use of Personal Protective Equipment (PPE) when necessary.
  • Utilize the ClearCare Telephony system for accurate clocking in and out of shifts, and update task statuses accordingly.
  • Follow the company dress code and arrive at work punctually, prepared for the day's responsibilities.
  • Engage in ongoing training to meet client care requirements and stay updated on company policies.
  • Participate in staff meetings and adhere to all company policies as outlined in the Employee Handbook.
Qualifications:
  • A minimum of 1,000 hours of experience in long-term care with elderly or chronically ill individuals, equivalent to approximately 6 months of full-time or 12 months of part-time work.
  • Preferred certification as a Certified Nursing Assistant (CNA) or Home Health Aide (HHA).
  • Current TB testing documentation.
  • Physical capability to perform job duties effectively.
  • Reliable transportation is essential; if client transportation is required, a valid driver's license and automobile insurance are necessary.
  • Strong communication skills and a caring, mature demeanor are crucial.
  • Successful completion of a criminal background check and provision of verifiable references.
Work Environment:

The Caregiver operates under the supervision of the Care Manager and receives scheduling guidance from the Staffing Coordinator. The position requires physical stamina to assist clients and travel to their residences.
