Position Overview

The Personal Support Specialist plays a vital role in assisting clients with their daily living activities as outlined in a personalized care plan developed by the agency's Care Manager.

Key Responsibilities:
  • Provide hands-on personal care services, along with companion and homemaker tasks, which may include light housekeeping, meal preparation, shopping, errands, transportation, and companionship.
  • Understand and implement the client's care plan effectively.
  • Assist clients with various needs, including companionship, meal preparation, light housekeeping, transportation, medication reminders, bathing, dressing, feeding, ambulation, and protective supervision.
  • Report any client concerns, changes in care, or emergencies to the Care Manager promptly.
  • Adhere to company Infection Control practices and notify the office if additional Personal Protective Equipment (PPE) or supplies are needed.
  • Utilize the ClearCare Telephony system to clock in and out for all scheduled shifts and accurately update task statuses for each client.
  • Follow the company dress code as specified in the Employee Handbook.
  • Complete ongoing training as required by client care needs and changes in company policies.
  • Attend staff meetings and adhere to all company policies and procedures.
Qualifications:
  • A minimum of 1,000 hours of experience in long-term care with elderly or chronically ill individuals, equivalent to approximately 6 months of full-time work or 12 months of part-time work.
  • Preferred certification as a Certified Nursing Assistant (CNA) or Home Health Aide (HHA).
  • Current proof of TB testing.
  • Ability to perform the physical duties required for this position.
  • Reliable transportation is essential; if transporting clients, a valid driver's license and automobile insurance are required.
  • Excellent communication skills and a mature, caring, punctual, and reliable demeanor.
  • Successful completion of a criminal background check.
  • Provision of verifiable professional and personal references.
Work Environment:

The Personal Support Specialist will be supervised by the Care Manager and will receive scheduling direction from the Staffing Coordinator.

Physical Demands:
  • Ability to reposition and transfer clients safely.
  • Capable of sustaining the weight of clients using proper transfer techniques and equipment.
  • Travel to clients' residences using reliable transportation.
