As the ACT Program Clinical Manager, you will be responsible for overseeing the day-to-day operations of the ACT team, ensuring high-quality clinical care, and fostering a collaborative and supportive work environment.
Responsibilities:- Direct the daily operations of the ACT team, including scheduling staff work hours to ensure comprehensive coverage for routine and on-call hours.
- Lead daily ACT organizational and clinical meetings; continuously evaluate the status of ACT consumers and coordinate treatment activities to address their evolving needs.
- Develop and maintain relationships with law enforcement agencies, human services organizations, and community resources to enhance service delivery.
- Provide direct ACT interventions for consumers, collaborating with team members to assist them in achieving their goals.
- Assign new consumers to ACT Team members, review intake information, and ensure each consumer has an Individual Service Plan (ISP) that is regularly updated and addresses their specific needs.
- Ensure all clinical documentation adheres to ACT interventions outlined in Chapter 34 and reflects medically necessary services provided.
- Serve as the Active Qualified Practitioner responsible for approving progress notes, Individual Recovery Plans, and Individual Specific Service Plans.
- Provide on-call duty for after-hours staff access and supervision, assisting with crisis situations and ensuring team coverage during scheduled and unscheduled working hours.
- Conduct quality assurance audits to review clinical content in records, ensuring recovery plans are in place, services are effective, re-hospitalizations are documented, and discharge planning is completed as needed.
- Train ACT team members on entitlements, benefits applications (including Medicaid), and the impact of income on housing and employment. Oversee appeals for denied benefits related to SSI/SSDI.
- Master's Degree required in a relevant field such as social work, psychology, or counseling.
- Minimum of 1-3 years of experience working with individuals with mental illnesses, providing services like crisis management, intensive case management, individual and/or family therapy, skill training, and rehabilitation supports.
- Current licensure in the District of Columbia is required.
- Two years of supervisory experience preferred; four years is highly desirable.
