SOME, Inc. is seeking a highly skilled Clinical Program Manager to oversee the Senior Services department. The ideal candidate will have a Master of Social Work (MSW) degree and be licensed in the District of Columbia (LGSW or LICSW). The Clinical Program Manager will provide program and administrative oversight of Kuehner Place, a 7-bed transitional housing program for older adults experiencing homelessness.
Key Responsibilities:
- Manage intake, enrollment, and intensive case management processes for prospective and current program participants.
- Participate as an active member of the clinical care team and provide clinical support and consultation to Kuehner House and Karin House residents and staff.
- Supervise night and front desk monitors, clinical interns, and other staff members as needed.
- Develop and implement care management plans with clients, including resource referrals and one-on-one support sessions.
- Collaborate with medical clinic staff to ensure the management of resident chronic health diagnoses.
- Ensure compliance with all state, local, and federal laws governing the workplace and department functions.
Requirements:
- Master of Social Work (MSW) degree and licensure in the District of Columbia (LGSW or LICSW).
- 2+ years of previous case management experience with senior and/or at-risk populations.
- Excellent verbal and written communication skills, with the ability to work independently and as part of a team.
- Proficient with MS Office, including Word, Excel, and Outlook.
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
