Bilingual Case Manager

Catholic Charities Community Services is seeking a highly skilled and compassionate Bilingual Case Manager to join our team. As a key member of our social services team, you will provide direct case management services to clients at risk of becoming homeless, advocating for their needs and connecting them with essential resources.
Key Responsibilities- Provide crisis intervention services to individuals and families facing housing crises, addressing emergencies related to housing, food, medical needs, and other concrete needs.
- Conduct thorough needs assessments and develop service plans to address identified needs using a short-term intervention model.
- Refer clients to Catholic Charities programs and/or clinics as deemed appropriate, ensuring seamless support and connection to resources.
- Assist clients in applying for and receiving public and private benefits, promoting self-sufficiency and stability.
- Maintain accurate documentation of services provided to clients, ensuring compliance with organizational standards.
- Develop and maintain effective communication with other programs and departments within the organization, facilitating inter-agency referrals and collaboration.
- Attend regular sessions with immediate supervisor, staying informed about program developments and best practices.
- Conduct educational and/or supportive groups, promoting community engagement and social support.
- Complete statistical reports, providing valuable insights into program effectiveness and areas for improvement.
- Bachelor's degree or at least two years' experience in the social services field.
- Bilingual, English/Spanish, a plus.
- Excellent interpersonal skills, knowledge of social service delivery systems, and knowledge of City resources.
- Good organizational, time-management, and communication skills.
- Ability to work well independently and as part of a team.

- Conduct home visits and outreach within NYC, using public transportation or traversing city streets, occasionally ascending/descending stairs, and being exposed to outside weather conditions.
- Ability to work with the public, position boxes/items at different levels that weigh up to 25 lbs, and remain stationary at a workstation using a computer at least 50% of the time.
- Potential to carry a laptop to offsite work locations.
