Job Summary

The Bilingual Case Management Coordinator is a key member of our team, responsible for providing case management services to constituents. This role requires strong communication and interpersonal skills, as well as the ability to work with diverse populations.

Key Responsibilities
  • Provide ongoing case management services to constituents, including writing case notes and identifying needs.
  • Support the scheduling of intakes and conduct intakes, including maintaining paper records and collecting demographic information.
  • Work and coordinate with front desk and administrative staff to ensure constituents move smoothly through the intake process.
  • Provide support for all Center Support teams and assist in linking constituents to internal and external resources.
Requirements
  • Two to four years of experience in case management, mental health, community, and social services support.
  • Bilingual or multilingual required, with language preferences including Spanish, Bengali, Russian, French, and/or French-Creole.
  • Excellent administrative, organization, and written/verbal communication skills.
  • Strong interpersonal skills and ability to work with a wide range of work styles and cultural backgrounds.
