Responsibilities:
  • Provide case management services, including benefits and entitlements, case assistance, and group programming
  • Conduct intakes, screenings, and needs assessments as assigned
  • Facilitate support groups and provide information and referrals to participants
  • Maintain accurate and timely case records in our STARS Peerplace database
  • Comply with all funder service standards and contract requirements
  • Participate in case presentations, staff meetings, and regular training
Requirements:
  • Bachelor's degree in human services, social work, or a related field
  • 1-2 years of experience in the health and human service field, including services in long-term services and supports
  • Knowledge of government entitlement and social services systems
  • Excellent organizational, communication, outreach, and advocacy skills
  • Proficiency in Google Suites, Microsoft Office, and data analysis
