Nurse Navigator for Gastrointestinal Health

Role Summary:
We are seeking a highly skilled Clinical Nurse Navigator RN to join our team in the Department of Int Med Gastroenterology PBC at Rush University Medical Center. As a key member of our interdisciplinary team, you will be responsible for providing exceptional care coordination, follow-up, and support to patients with gastrointestinal conditions.

Responsibilities:
• Maintains and ensures compliance of all policies and procedures that guide and support assigned services
• Provides consultation in specialty area to all levels of healthcare providers throughout the organization, affiliated schools of nursing, and the community
• Offers guidance and emotional support to patients regarding decisions on follow-up and treatment plans
• Arranges appointments, reviews test preparation procedures, and assesses potential patient barriers
• Contacts patients via phone calls, emails, and letters to remind them about appointments and answers questions about testing and treatment
• Performs follow-up phone calls to patients after appointments as indicated
• Works with participating sites to develop mechanisms for sharing patient data in a manner that optimizes their time and effort
• Completes electronic records and paperwork regarding patient interactions and appointments

Salary Range: $120,000 - $150,000 per year.
