Salary: $90,000 - $120,000 per year

About the Role:
The Community Nurse Navigator will work closely with safety net hospitals to provide comprehensive case management services to patients who need a diagnostic follow-up after their screening mammogram and those diagnosed with breast cancer. The ideal candidate will have experience in oncology nursing, case management, or patient navigation, as well as strong interpersonal and communication skills.

Responsibilities:
• Oversee the case management of patients who need a diagnostic follow-up and/or those diagnosed with breast cancer.
• Assess patients for social determinants of health impacting their care.
• Coordinate and schedule medical appointments for patients.
• Advocate for patients' healthcare needs and preferences within the healthcare system.

Requirements:
• Registered Nurse (RN) license in the state of Illinois.
• Bachelor of Science in Nursing (BSN) preferred.
• Experience in oncology nursing, case management, or patient navigation.
• Knowledge of breast cancer diagnosis, treatment, and support services.
• Strong interpersonal and communication skills.
• Cultural competence and sensitivity to diverse populations.
• Ability to work independently and collaborate with a multidisciplinary team.
• Proficient in medical documentation and electronic health records.
