Job Summary

We are seeking an experienced Clinical Nurse Navigator RN - Gastroenterology to join our team. In this role, you will provide care coordination and support to patients, enhancing their overall experience and promoting positive health outcomes.

Responsibilities

  1. Maintain and ensure compliance with policies and procedures guiding our services.
  2. Provide consultation and guidance on specialty areas to healthcare providers.
  3. Patient advocacy: offer emotional support and facilitate decision-making regarding follow-up and treatment plans.
  4. Arrange appointments, review test preparation procedures, and assess potential barriers to ensure successful testing processes.
  5. Contact patients via phone calls, emails, and letters to remind them about appointments and address questions about testing and treatment.
  6. Perform follow-up phone calls to patients as needed.
  7. Work with participating sites to develop mechanisms for sharing patient data.
  8. Complete electronic records and paperwork regarding patient interactions and appointments.
  9. Develop relationships with community leaders and organizations to promote health education and outreach.

Requirements and Qualifications

To excel in this role, you must possess:

  • Current State of Illinois Registered Nurse licensure
  • Bachelor's degree in Nursing or Master's degree in Nursing
  • At least three years of registered nurse experience
  • BLS certification
  • Excellent written and verbal communication skills
  • Interpersonal skills and ability to work independently and as part of a team

Estimated Salary
