**Responsibilities**

  • Monitor Admission Discharge Transfer (ADT) feed daily to ensure timely follow-up with members and facilitate post-discharge appointments.
  • Develop and implement educational programs to promote care coordination and its importance among MFS staff and community providers.
  • Collaborate with multidisciplinary professionals to provide a continuum of coordinated care to meet the complex needs of members.

  • Salary range of $47,000 to $52,000 annually, based on skill and experience level.
