Job Title: Case Manager - Street Outreach

About the Role:

Newport Mental Health is seeking a motivated and empathetic Case Manager - Street Outreach to join our team. As a key point of contact with clients, you will be responsible for developing strategies for engaging clients on the streets and developing a path to housing or shelter with the support of community partners.

Key Responsibilities:

  • Provide street-based outreach, navigation, and supportive services to individuals experiencing homelessness.
  • Support clients in accessing resources in the community, establishing and maintaining professional relationships with staff, clients, and community providers.
  • Adhere to housing first and harm reduction models of advocacy with clients, conducting interactions with clients and other professionals in a respectful and courteous manner.
  • Listen attentively to client needs and suggestions, addressing their issues fairly and professionally, and coordinating with the immediate supervisor for supervision.
  • Develop and maintain strong ties to the community, law enforcement, and other homeless service providers.
  • Link clients to appropriate bridge/interim housing programs and assist them in completing required paperwork to enroll them in permanent housing-related programs.

Requirements:

  • 2 years minimum experience providing street-based services to highly vulnerable populations.
  • Street-based outreach preferred.
  • Exceptional communication skills with the ability to professionally communicate verbally and in writing with a variety of individuals.
  • Caring and empathetic, with the desire to help others in need.
  • Self-motivated and independent, with the ability to prioritize work to meet deadlines.
  • Valid Driver's License with reliable transportation to travel to our facilities throughout the area.
