Position Overview

Role Summary:

The Admissions Nurse plays a crucial role in representing Hospice services to patients, families, and stakeholders through comprehensive pre-assessments and information dissemination. This position collaborates with the intake team, referring physicians, and other sources to uphold the hospice philosophy of care.

Key Responsibilities:
1. Adhere to the Nurse Practice Act and professional nursing standards in all duties.
2. Provide pre-assessment and admission services to referred patients, conducting necessary visits in various settings.
3. Maintain communication with the Intake Department and referring physicians to ensure the appropriateness of referrals and obtain necessary certifications.
4. Keep the Intake Department updated on referral and admission progress.
5. Apply nursing principles to deliver care, emotional support, and education to patients and their families, encouraging active participation in care plans.
6. Utilize computer skills for accessing medication information and documenting in electronic medical records.
7. Foster teamwork by actively participating as a member of an interdisciplinary team, sharing essential patient information for coordinated care.
8. Collaborate with other disciplines and community resources to meet patient care needs efficiently and responsibly.
9. Complete documentation in accordance with agency policies, ensuring clarity and completeness.
10. Provide hands-on nursing care to a diverse patient population as assigned.
11. Stay updated on hospice care regulations and integrate knowledge into clinical practice.
12. Participate in weekend, holiday, and on-call coverage as required.

Additional Responsibilities:
This position may include, but is not limited to, the following tasks:
1. Assess and respond to patient situations promptly, administering emergency care as necessary.
2. Share expertise through collaboration and in-service training.
3. Engage with patients and families from diverse backgrounds while maintaining professional boundaries.
4. Exemplify excellent customer service in all interactions.
5. Represent MorseLife Health Systems positively within the community.
6. Exhibit effective listening skills in communications.
7. Meet continuing education requirements.
8. Participate in daily reports, interdisciplinary meetings, case conferences, and quality assurance activities.
9. Suggest improvements for policies and procedures as appropriate.
10. Adhere to safety and OSHA policies, reporting unsafe practices promptly.
11. Participate in departmental projects and committees as needed.
12. Maintain a positive attitude towards the agency's mission and values.

Qualifications:

  • Education: Graduate of an accredited nursing program.
  • Experience: Minimum of two years nursing experience required; preferred experience in intake, home care, hospice, or oncology nursing.
  • License/Certification: Registered Nurse licensure in Florida; valid driver's license with insured automobile.

  • Ability to interact with patients and families of all ages.
  • Strong assessment and communication skills.
