Position Overview

Role Summary:

The Admissions Nurse plays a crucial role in representing Hospice services to patients, families, and other stakeholders through comprehensive pre-assessments, information dissemination, and admissions processes, whether in a patient's residence or a healthcare facility. This position collaborates closely with the intake team, referring physicians, and various referral sources to uphold the hospice philosophy of care.

Key Responsibilities:
Conducts all tasks in alignment with the Nurse Practice Act and adheres to the fundamental principles of professional nursing.
Delivers pre-assessment and admission services to patients referred to hospice, including home or facility visits as required.
Maintains communication with the Intake Department, referring physicians, and the Hospice Medical Director to ensure the suitability of each referral and secure necessary certifications.
Updates the Intake Department on the status of referrals and admissions.
Employs nursing principles and techniques in patient treatment, providing care, emotional support, and education to patients and their families. Encourages active participation from patients and family members in the care plan to achieve established objectives.
Utilizes computer skills to access medication information and document care in the electronic medical record as mandated.
Fosters teamwork by engaging actively as a member of an interdisciplinary team, sharing essential patient information to coordinate care effectively.
Collaborates with other interdisciplinary team members to ensure efficient and financially responsible care coordination.
Makes timely referrals for additional services and medical supplies to meet patient care needs.
Completes and submits documentation promptly, ensuring clarity and completeness that accurately reflects the care provided.
Provides direct nursing care to a diverse patient population as assigned.
Stays informed about hospice care regulations and reimbursement guidelines, integrating this knowledge into clinical practice.
Participates in weekend, holiday, and on-call duties as required.

Exhibiting flexibility and a positive attitude towards additional responsibilities.
Attending mandatory meetings and training sessions unless excused in advance.

Qualifications:
Education: Completion of an accredited nursing program.
Experience: A minimum of two years of nursing experience is required; experience in intake, home care, hospice, or oncology nursing is preferred.
License/Certification: Registered Nurse licensure in the State of Florida; valid driver's license with a properly insured vehicle.
