As a vital member of the hospice care team, nurses deliver compassionate end-of-life support to enhance the comfort and quality of life for patients. This role requires working in a collaborative environment, emphasizing effective communication with patients, families, and other healthcare professionals. The ideal candidate is detail-oriented, patient, and possesses a calm demeanor. Hospice nurses are empathetic individuals with a genuine passion for helping patients navigate their final journey.
Responsibilities:
Operational Hours: Thursday - Sunday, 9am - 7:30pm
Key Duties:
- Facilitate care coordination for patients and families under the guidance of the primary physician and registered nurse.
- Identify and assess the needs of patients and families.
- Collaborate with the interdisciplinary group (IDG) to develop and implement personalized care plans.
- Employ monitoring, intervention, and educational skills to enhance patient comfort and quality of life.
- Commit to prioritizing the comfort and care of patients and families.
- Deliver exceptional, personalized service to patients and families consistently.
- Address and resolve any concerns from patients and families promptly and effectively.
Qualifications:
- Education/Regulatory Requirements:
- Current licensure as a Licensed Practical Nurse in the State of Florida.
- Current Basic Life Support Certification.
- 30-hour IV Certification preferred.
- Hospice Certification preferred.
- Skills:
- Ability to apply principles of hospice nursing, case management, crisis intervention, effective communication, and needs assessment.
- Reliable transportation with valid insurance and a Florida driver's license.
- Professional Requirements:
- Capability to collaborate with volunteers and healthcare professionals in delivering palliative care to hospice patients.
- Proficiency in performing nursing techniques and procedures under the supervision of a Registered Nurse.
- Language Skills:
- Bilingual (English/Spanish) preferred.
