About the Role
We are seeking a compassionate and skilled Hospice Nurse to join our team at Trustbridge. As a Hospice Nurse, you will play a vital role in providing end-of-life care to patients and their families, ensuring their comfort and quality of life.
Responsibilities
- Evaluate patients referred for hospice services and develop individualized care plans.
- Collaborate with healthcare professionals, including physicians, hospitals, and long-term care facilities, to coordinate patient care.
- Market the hospice concept to healthcare professionals and the community through education and outreach.
- Provide emotional support and guidance to patients and their families, addressing their physical, emotional, and spiritual needs.
Requirements
- Florida licensed R.N. with a B.S.N. preferred.
- 1 year medical-surgical experience preferred.
- Preferred CHPN Certification.
- Excellent verbal and written communication skills, with the ability to assess patient and family needs.
- Organized, flexible, and self-motivated, with the ability to work independently and as part of a team.
