About the Role
We are seeking a compassionate and skilled Hospice Nurse to join our team at Trustbridge. As a Hospice Nurse, you will play a vital role in providing end-of-life care to patients and their families.
Key Responsibilities
- Conduct thorough evaluations of patients referred for hospice services, ensuring accurate assessments and effective care planning.
- Collaborate with healthcare professionals, including physicians and hospitals, to coordinate patient care and treatment plans.
- Act as a liaison between Trustbridge and healthcare providers, facilitating communication and ensuring seamless care transitions.
- Market the hospice concept to healthcare professionals and the community, promoting education and awareness about our services.
Requirements
- Education/Regulatory Requirements: RN degree from an accredited college or university, current licensure in the State of Florida, and Basic Life Support Certification.
- Skills: Excellent communication and case management skills, with the ability to work effectively in a team environment. Bilingual (English/Spanish) preferred.
- Professional Requirements: Ability to work collaboratively with healthcare providers, volunteers, and patients, with a focus on delivering high-quality patient care.
