Hospice Intake Registered Nurse

Role Summary:
The Admissions Nurse plays a crucial role in representing Hospice services to patients, families, and stakeholders through comprehensive pre-assessments and admissions, whether in a patient's residence or a healthcare facility. This position collaborates closely with the intake team, referring physicians, and other sources to uphold the hospice philosophy of care.
Key Responsibilities:
1. Adhere to the Nurse Practice Act and professional nursing standards in all duties.
2. Provide pre-assessment and admission services to referred patients, conducting necessary visits in their homes or healthcare settings.
3. Communicate effectively with the Intake Department, referring physicians, and the Hospice Medical Director to ensure the appropriateness of referrals and obtain necessary certifications.
4. Keep the Intake Department updated on the status of referrals and admissions.
5. Apply nursing principles in patient treatment, offering care, emotional support, and education to patients and their families. Encourage active participation in the care plan to achieve set goals.
6. Utilize computer skills to access medication information and document in electronic medical records as required.
7. Foster teamwork by actively participating as a member of an interdisciplinary team, sharing vital patient information to coordinate care.
8. Collaborate with various disciplines and community resources to ensure timely and efficient patient care while being fiscally responsible.
9. Complete and submit documentation in accordance with agency policies, ensuring clarity and completeness.
10. Provide hands-on nursing care to a diverse patient population as assigned.
11. Stay updated on hospice care knowledge, including regulatory and reimbursement guidelines, and integrate this knowledge into clinical practice.
12. Participate in weekend, holiday, and on-call coverage as needed.
Additional Responsibilities:
This role encompasses various responsibilities, including but not limited to:
1. Assessing and responding promptly to patient situations, administering emergency care as necessary.
2. Sharing expertise through in-service training and collaboration.
3. Working with patients and families from diverse backgrounds while maintaining professional boundaries.
4. Demonstrating a caring attitude towards clients and colleagues.
5. Upholding excellent customer service standards in all interactions.
6. Representing MorseLife Health Systems positively within the community.
7. Exhibiting effective listening skills during communications.
8. Meeting continuing education requirements.
9. Participating in daily reports, interdisciplinary meetings, case conferences, and quality assurance activities.
10. Contributing suggestions for policy and service improvements.
11. Practicing safe working techniques and reporting unsafe conditions promptly.
12. Understanding and supporting agency safety and compliance policies.
13. Reporting incidents according to agency protocols.
14. Engaging in departmental projects and committees as required.
15. Demonstrating flexibility and a positive attitude in adapting to additional duties.
16. Attending mandatory meetings and training sessions, unless excused in advance.
Qualifications:
- Education: Graduate of an accredited nursing program.
- Experience: Minimum of two years of nursing experience required; experience in intake, home care, hospice, and/or oncology nursing preferred.
- License/Certification: Registered Nurse licensure in the State of Florida; valid driver's license with an insured automobile in good working order.
