Hospice nurses play a vital role in providing compassionate end-of-life care, ensuring comfort and enhancing the quality of life for patients. They work within collaborative, team-oriented settings that emphasize ongoing communication with patients, their families, and other healthcare providers. The ideal candidate is meticulous, patient, and composed, embodying the empathy and kindness essential for supporting patients during this significant phase of their lives.
Key Responsibilities
Work Schedule: 12pm - 10:30pm and alternating weekends
Primary Duties
- Responsible for assessing all patients referred for hospice services.
- Clearly communicates the organization's continuum of care and facilitates appropriate referrals for patients and families.
- Collaborates with referring physicians and designated liaisons to coordinate care plans for hospice-eligible patients and their families.
- Acts as the primary contact between Trustbridge and healthcare professionals, including physicians, hospitals, and long-term care facilities.
- Promotes the hospice philosophy through educational outreach to healthcare providers and the broader community.
Required Qualifications
A. Educational and Regulatory Standards:
- Must hold a valid Florida RN license; BSN is preferred.
- At least one year of medical-surgical nursing experience is preferred.
B. Skills and Competencies:
- CHPN Certification is preferred.
- Ability to assess both the physical and psychosocial needs of patients and their families; hospice experience is highly desirable.
- Excellent verbal and written communication skills are essential; bilingual candidates are preferred. Experience with electronic medical record systems is a plus.
- Must be organized, adaptable, and self-motivated, with the ability to engage effectively with all levels of the healthcare team.
- Strong customer service skills are necessary, along with the ability to work independently with minimal supervision.
