Key Responsibilities:

  • Facilitate participant-centered planning and service delivery, ensuring effective implementation.
  • Identify and coordinate access to necessary long-term services and supports (LTSS) and other covered services.
  • Provide essential information to participants, assisting them in locating and coordinating required services.
  • Educate participants about LTSS assessments, planning processes, and available service options.
  • Support participants in understanding their rights and navigating the complaint and grievance processes.
  • Gather vital information regarding participant preferences and goals to aid in service planning.
  • Conduct annual reevaluations of care levels in compliance with CHC standards.
  • Assist participants in completing activities to maintain LTSS eligibility.

Qualifications:

  • A Bachelor's Degree in social work, psychology, or a related field, or a minimum of three years of experience in a social service or healthcare environment.
  • Experience in social services or healthcare is preferred.
  • A valid driver's license is required.
  • Proficiency in Microsoft Office, particularly Excel, is highly desirable.
  • Experience working with individuals with disabilities or seniors requiring LTSS is beneficial.
  • Familiarity with home and community-based service systems and access processes.
  • Experience in conducting LTSS needs assessments and monitoring service delivery.
  • Cultural competence and the ability to advocate effectively for participants.
  • Professional interaction skills with healthcare professionals.
  • Willingness to travel locally within the assigned community.
