Job Summary

RiseBoro Community Partnership is seeking a skilled Case Manager to join our team. As a Case Manager, you will be responsible for providing housing assistance and holistic case management services to program participants. Your goal will be to secure long-term housing stability for our clients.

Key Responsibilities
  • Coordinate services and activities to provide program participants with necessary tools to sustain their housing and prevent homelessness
  • Work with clients, household members, supervisors, and supplementary staff to execute the service plan
  • Establish goals and actions that address the immediate housing crisis and the underlying causes of the crisis
  • Monitor and evaluate program participants' progress on the implementation of their service plan
  • Prepare all necessary documentation to prepare in-house subsidy applications on behalf of the client
  • Provide referrals for clients to access community supports, public benefits, and entitlements
Requirements
  • Associate degree required plus three years of experience; Bachelor's degree preferred
  • Bilingual Spanish
  • Experience in customer or client services required
  • Ability to problem solve and work with a high level of autonomy required
