Job Summary

We are seeking a skilled Case Manager to join our team at Institute for Comm Living. The successful candidate will be responsible for organizing comprehensive service planning and assessment activity, ensuring service plan implementation through direct case management or co-provider services.

Key Responsibilities
  • Participate in orientation of new residents, explaining available clinical services, recreational activities, and ICL programs.
  • Obtain and document assessment information, including medical, psychological, and social factors contributing to the resident's situation.
  • Collaborate with residents to complete and maintain resident service plans, incorporating an Integrated Healthcare approach.
  • Provide supportive counseling, instruct residents in daily living skills, and facilitate socialization skill enhancement and conflict resolution.
  • Obtain and develop resource information for residents, disseminating it in a clear and understandable manner.
  • Use cognitive-behavioral techniques to assist residents in meeting service plan goals.
  • Facilitate individualized services, perform crisis assessment and nonverbal and verbal crisis intervention, and meet with residents weekly to follow up on service plan goals.
  • Refer residents to community resources, accompany them on visits to medical treatment facilities, and provide transportation for service plan-related purposes.
  • Assist residents with developing savings and money management plans, offer instruction in budgeting, banking, and credit, and plan social or recreational activities.
  • Advocate on behalf of residents with outside service providers and within ICL.
Requirements
  • Bachelor's degree in a relevant field, including a practicum or major in social work, psychology, nursing, or a related field, and one year of case management experience.
  • Associate's degree in a health or human services field and three years of case management experience.
  • High school diploma/GED and three years of experience in providing direct services to individuals with mental disabilities or homeless individuals.
