Job Title: Case Manager

We are seeking a highly motivated and compassionate Case Manager to join our team at Women In Need, Inc. The successful candidate will be responsible for providing case management services to clients in transitional housing, ensuring their successful integration into both housing and employment goals.

Key Responsibilities:
  • Conduct initial assessments of clients within 48 hours of arrival and complete necessary documentation.
  • Initiate referrals for emergency services and ensure timely completion of intake information.
  • Provide comprehensive case management, including crisis intervention, counseling services, and support.
  • Develop and track progress against Independent Living Plans (ILPs) for each client.
  • Collaborate with housing coordinators to ensure clients are compliant with seeking permanent housing.
  • Attend shelter meetings, agency-wide meetings, and staff training as directed by the supervisor.
Requirements:
  • Bachelor's Degree in social work or related field preferred, with two years of relevant experience or an Associate's Degree and three years of client-facing experience.
  • Familiarity with Evidence-Based Practices (EBP) and entitlement systems.
  • Excellent organizational, written, and verbal communication skills.
  • Ability to work effectively in a team environment.
