Substance Use Disorder (SUD) Clinical Care Specialist

We are seeking licensed mental health professionals to join our Care Navigation team (LCSW, LMFT, LPC required) to assist members in need of substance use support. The SUD Clinical Care Specialist will conduct initial evaluations, provide support, facilitate referrals, and ensure ongoing care coordination for members engaged in Spring Health's innovative SUD program.

This role operates on a standard schedule, Monday through Friday, from 9 AM to 5 PM PST.

Key Responsibilities

  • Conduct telephonic assessments, brief interventions, and refer members to appropriate treatment providers (utilizing the SBIRT model).
  • Employ active listening, psycho-education, and motivational interviewing techniques to comprehend each member's requirements and assist them in establishing goals to reduce substance use and/or achieve successful recovery.
  • Utilize ASAM criteria to determine suitable levels of care, facilitate referrals, and coordinate care with Spring Health's SUD program or providers.
  • Offer consultations regarding mental health issues and family/relationship dynamics, as needed based on member requirements.
  • Provide immediate support and safety planning for members facing safety risks or crises, as necessary.
  • Communicate effectively with members and providers through written correspondence.
  • Maintain accurate documentation, records, and follow-ups, tracking progress across various systems, including an EHR, within required timelines.
  • Identify and address barriers or disruptions to care.
  • Collaborate with multi-disciplinary teams, including care support, provider operations, and benefits/billing, to ensure a seamless member experience.
  • Engage in staff meetings, case discussions, and training sessions.
  • Assist in onboarding and mentoring new team members.
  • Adhere to Spring Health policies while maintaining confidentiality, compliance, and ethical standards.

Qualifications

  • Required mental health license: LCSW, LMFT, LPC.
  • At least 2 years of experience in SUD clinical care.
  • Familiarity with motivational interviewing, SBIRT, and triage processes.
  • Knowledge of ASAM criteria, contingency management, and medication-assisted treatment (MAT).
  • Exceptional listening, written, and verbal communication skills.

Preferred Qualifications

  • Licensure or certifications in SUD.
  • Bilingual capabilities.
  • Willingness to work flexible hours.

The target base salary for this position is $42.31 per hour, part of a competitive total rewards package that includes stock options and benefits. Individual compensation may vary based on experience, location, internal equity, and other relevant business factors.
