The Role of Care Transition Navigator

As we expand our services, we are looking for a Care Transition Navigator who embodies our core values: Trustworthiness, Capability, Compassion, Proactivity, and a Calling to serve patients in their homes. This role serves as the primary point of contact for referral sources, ensuring a seamless transition for patients returning home. Your efforts will help position VitalCaring as the preferred partner for compassionate and personalized home health care.

Compensation and Benefits

Experience Required
  • Active RN license, valid state driver's license, and reliable transportation
  • Minimum of two years of clinical experience; one year in home health or healthcare sales preferred
  • Understanding of business management, regulatory requirements, and accreditation standards
