The Transition Oversight Specialist (TOS) operates under the general guidance of the Admissions/Placement Coordinator.
Responsibilities include delivering routine consultative services and offering technical assistance during the monitoring of individuals transitioning from a State Supported Living Center (SSLC) to a community environment.
The TOS aids in formulating supports and services as part of the transition planning process.Engaging with individuals and community provider staff to assess the effectiveness of the supports and services outlined in the transition plan is essential.
The TOS prepares a comprehensive report and informs SSLC staff of any issues identified.Facilitates resolution of disputes or dissatisfaction with community supports and services post-transition and collaborates to develop solutions for any concerns.
This position requires moderate supervision, allowing for limited initiative and independent judgment. Travel may be necessary up to 50% of the time.Key Responsibilities:
Regular attendance is essential, and the role may require adherence to a specific shift schedule or, at times, a rotating schedule, extended shifts, and/or overtime as per agency leave policy. Additional duties may be assigned as needed.
May participate in an on-call status on a rotating basis to meet operational needs.Provides assistance and collaboration in planning and developing community supports as part of the transition planning process for individuals moving from the SSLC to a community setting.
Schedules, coordinates, and executes on-site monitoring of individuals following their transition from the SSLC to a community setting.
Monitors the availability of supports and the delivery of services specified in the Community Living Discharge Plans (CLDP) for each individual.
Engages with individuals in their home and day program settings at 7, 45, and 90-day intervals, or more frequently as required.
Completes monitoring checklists after each visit and communicates findings with the Admission/Placement Coordinator and the interdisciplinary team as necessary to address any identified issues.
Facilitates resolution of disputes or dissatisfaction with community supports and services post-transition and collaborates to develop solutions for any concerns.
Additional responsibilities may include actively participating and/or serving in a supporting role to fulfill the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation.
Such participation may necessitate an alternate shift pattern assignment and/or location.Required Knowledge, Skills, and Abilities:
Proficient in preparing reports and communicating effectively both orally and in writing.
Ability to engage professionally with individuals with intellectual disabilities, family members, staff, community providers, and others.
Familiarity with Health and Human Services Commission (HHSC) community programs, including ICF/IID and waiver programs.
Ability to gather, organize, correlate, and analyze information and devise solutions to challenges.
Proficient in computer applications such as WORD, EXCEL, and other Microsoft Office products.
Capable of managing multiple responsibilities and planning work activities effectively.
Willingness to travel up to 50% of the time.
