About the Role:

The University of Vermont Health Network is seeking a skilled Clinical Resource Coordinator - LPN to join our team. As a Clinical Resource Coordinator - LPN, you will work under the general direction of the AVP Population Health Services and collaborate closely with Population Health Services Organization (PHSO) and Medical Group teams to support High Value Care (HVC) performance initiatives and centrally supported clinical workflows.

Key Responsibilities:

  • Maintain working knowledge of HVC and PHSO strategy and primary care clinical workflows.
  • Work with the Network AVP, Population Health Services to support prioritized and targeted initiatives and provide the PHSO a more direct way to support HVC performance by augmenting clinic-based care teams and clinical workflows as appropriate.
  • Gather and organize patient data to identify patients' unmet needs, engage patients and care teams in care, and enhance ongoing communication and coordination with care teams.

Requirements:

  • Graduate of an accredited Licensed Practical Nursing Program.
  • One to two years of experience in a relevant healthcare setting, with primary care experience preferred.
