About the Role

The University of Vermont Health Network is seeking a skilled Clinical Resource Partner to join our team. As a key member of our Population Health Services Organization, you will play a vital role in supporting High Value Care (HVC) performance initiatives and centrally supported clinical workflows.

Key Responsibilities

  • Maintain working knowledge of HVC and PHSO strategy and primary care clinical workflows.
  • Work closely with Population Health Services Organization (PHSO) and Medical Group teams to support HVC performance initiatives.
  • Provide clinical support to augment clinic-based care teams and clinical workflows as needed.
  • Gather and organize patient data to identify patients' unmet needs and engage patients and care teams in care.

Requirements

  • Graduate of accredited Licensed Practical Nursing Program.
  • Current LPN licensure or compact licensure recognized by the State of Vermont.
  • One to two years of experience in a relevant healthcare setting, with primary care experience preferred.
