Case Manager

About the Role:
Radias Health is seeking a highly skilled and compassionate Case Manager to join our Targeted Case Management team. As a Case Manager, you will play a vital role in providing person-centered services to adults with serious persistent mental illnesses and co-occurring substance use disorders in Ramsey County.
Key Responsibilities:
- Develop and implement treatment plans based on diagnostic and functional assessments, approved by clinical supervisors.
- Ensure compliance with Minnesota Rule Chapter 9520 and provide access to necessary medical, vocational, social, educational, financial, legal, and housing services.
- Develop Individual Community Support Plans for each client every 6 months.
- Schedule face-to-face visits as dictated by the ICSP.
- Make culturally sensitive referrals and assist clients in accessing needed services.
- Participate in the commitment process, provide testimony in probate court, and write required court reports.
- File legally required reports with Vulnerable Adults, the Ombudsman, and Child Protection.
- Assume responsibility for providing services to maintain and enhance each client's ability to live independently.
- Develop individualized emergency support plans coordinated with community emergency support services.
- Assist clients with housing issues and interface with HRA and PHA staff, as well as landlords.
- Attend scheduled quarterly reviews and treatment planning meetings.
- Collaborate with family members and others interested in the client's treatment and welfare when warranted.
- Provide education to clients regarding their illness.
- Provide team and agency coverage of client needs.
- Develop therapeutic relationships with clients.
- Complete all paperwork, reports, and charting contemporaneously and in an organized manner.
- Participate in ongoing professional growth and development, maintain professional behavior, and participate in supervision.
- Transport clients in the community to various appointments.
Requirements:
- Bachelor's degree in behavioral sciences or related field, with 2,000 hours of supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders - OR -
- Masters or other graduate degree in behavioral health sciences or related field - OR -
- If no degree, 4,000 hours of supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders.
- Must have a car, auto insurance, and a driver's license.
