Job Title: Case Manager

About the Role:

Radias Health is seeking a highly skilled and compassionate Case Manager to join our Targeted Case Management team. As a Case Manager, you will play a vital role in providing person-centered services to adults with serious persistent mental illnesses and co-occurring substance use disorders in Ramsey County.

Key Responsibilities:

  • Develop and implement treatment plans based on diagnostic and functional assessments, approved by clinical supervisors.
  • Ensure compliance with Minnesota Rule Chapter 9520 and provide access to necessary medical, vocational, social, educational, financial, legal, and housing services.
  • Develop Individual Community Support Plans for each client every 6 months.
  • Schedule face-to-face visits as dictated by the ICSP.
  • Make culturally sensitive referrals and assist clients in accessing needed services.
  • Participate in the commitment process, provide testimony in probate court, and write required court reports.
  • File legally required reports with Vulnerable Adults, the Ombudsman, and Child Protection.
  • Assume responsibility for providing services to maintain and enhance each client's ability to live independently.
  • Develop individualized emergency support plans coordinated with community emergency support services.
  • Assist clients with housing issues and interface with HRA and PHA staff, as well as landlords.
  • Attend scheduled quarterly reviews and treatment planning meetings.
  • Collaborate with family members and others interested in the client's treatment and welfare when warranted.
  • Provide education to clients regarding their illness.
  • Provide team and agency coverage of client needs.
  • Develop therapeutic relationships with clients.
  • Complete all paperwork, reports, and charting contemporaneously and in an organized manner.
  • Participate in ongoing professional growth and development, maintain professional behavior, and participate in supervision.
  • Transport clients in the community to various appointments.

Requirements:

  • Bachelor's degree in behavioral sciences or related field, with 2,000 hours of supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders - OR -
  • Masters or other graduate degree in behavioral health sciences or related field - OR -
  • If no degree, 4,000 hours of supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders.
  • Must have a car, auto insurance, and a driver's license.
