Job Summary:
The Hybrid Case Manager is a critical role at Phoenix Service Corporation, responsible for coordinating services for vulnerable individuals and assisting them in navigating complex service delivery systems. This position requires a unique blend of skills, including knowledge of disabilities and related conditions, excellent interpersonal relationship skills, and the ability to work effectively with clients, families, guardians, service providers, and other members of the interdisciplinary team.
Key Responsibilities:
- Assist clients in locating and gaining access to necessary services, including medical, social, educational, and other needed services.
- Maintain knowledge of Hennepin and Ramsey County processes, policies, and systems.
- Provide coverage for cases when necessary in both Hennepin and Ramsey County, ensuring a manageable and full-time caseload.
- Ensure informed choice for all individuals, coordinating person-centered plans of care that balance what is important to the person with what is important for the person based on their needs and preferences.
- Monitor services to ensure the client's health and safety are assured, conducting face-to-face visits every six months or as needed.
- Assist in the identification of potential service providers and plan for/arrange services, informing county waiver contact of proposed waiver services to be authorized for entry into MMIS.
- Respond to all Minnesota Adult Abuse Reporting Center (MAARC) reports in a timely manner, documenting all responses and available for review upon request.
- Function as a role model, communicating effectively with clients, families, co-workers, and outside agencies, verbally and in written format, listening accurately and responding appropriately.
- Display positive, supportive, and cooperative conduct, maintaining and fostering cordial relationships and optimal communication.
- Adhere to and maintain knowledge of various rules, regulations, and state and county policies regarding the CADI, DD, and BI waiver programs, Rule 185 Targeted Case Management, and all policies and procedures/rules and regulations of Phoenix Service Corporation and its governing agencies.
- Complete all necessary documentation, developing systems that provide continual improvement in quality services.
- Monitor and respond to emergencies according to established policies and guidelines, maintaining an injury-free workplace by identifying and reporting unsafe working conditions.
- Participate in ongoing education in topics regarding disability case management services, attending and completing all annual in-service requirements.
Qualifications:
- Public Health Nurse, Registered Nurse, or Graduate of an accredited four-year college with a major in social work, psychology, sociology, or a closely related field; OR a graduate of an accredited four-year college with a major in any field and one year of experience as a social worker in a public or private social service agency.
- Excellent communication and interpersonal skills.
- Excellent problem-solving and organizational skills.
- Telephone, computer, and communication skills.
- Ability to read, write, and speak the English Language to adequately interact and respond to the individuals and meet their service needs.
- Ability to use a computer for required documentation, communication, and time log purposes.
- Ability to see and hear adequately to respond to emergency situations according to established Phoenix Service Corporation policies and procedures.
- Ability to travel to meet with clients and attend meetings, and if driving, holds a valid driver's license and maintains a safe driving record.
- Ability to sit, stand, reach, and bend.
Requirements:
- This position requires each employee to work from the office during their first three months of employment. At that time, their supervisor will evaluate if the individual can move into a hybrid/remote position if that is what they choose.
