About the Role:We are seeking a compassionate and skilled Registered Nurse to join our team as a Case Manager. As a Case Manager, you will be responsible for planning, organizing, and directing hospice care for patients and their families. You will work closely with our interdisciplinary team to provide comprehensive care and support, ensuring that our patients receive the highest level of care and comfort.
Key Responsibilities:- Conduct initial and ongoing assessments of patients and families to determine their needs and develop individualized care plans.
- Provide professional nursing care, including administering medications, treatments, and other interventions as prescribed by physicians.
- Collaborate with patients, families, and healthcare providers to ensure seamless care coordination and communication.
- Participate in interdisciplinary team meetings to discuss patient care and develop strategies for improving patient outcomes.
- Supervise and delegate tasks to hospice aides as needed.
- Graduate of an accredited school of nursing with a current license to practice in the state.
- One to two years of recent acute care experience in an institutional setting, with one to two years of recent experience in home care preferred.
- Excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
- Prolonged or considerable walking or standing, ability to lift, position, and/or transfer patients, and ability to lift supplies and equipment.
