About the RoleThe Case Manager III will provide comprehensive case management services, including intake, assessment, goal setting, and information and referral services, to older adults experiencing homelessness. The successful candidate will work closely with clients to identify and address barriers to housing and other goals, and provide ongoing support and aftercare.
Key Responsibilities- Provide comprehensive case management services, including intake, assessment, goal setting, and information and referral services.
- Assess clients' needs and develop person-centered care plans to address housing and other goals.
- Provide short-term counseling, skill building techniques, and aftercare to clients.
- Document client interactions and progress in a timely and accurate manner.
- Make referrals to other professionals and coordinate joint efforts to address clients' needs.
- Preferred: Minnesota Board of Social Work Approved License (e.g. LSW, LGSW,LISW, LICSW).
- One (1) year of broad-based clinical experience preferred.
- Ability to work with diverse and/or low-income populations, including those who may be experiencing homelessness, unstable housing, and/or mental illness required.
- CPR/First Aid Certification required.
