Key Responsibilities
The Care Coordinator collaborates with the healthcare team to provide ongoing support and communication for patients with chronic kidney disease (CKD) and End-Stage Renal Disease (ESRD).

This role serves as a primary contact to streamline resources across the care continuum, identify service gaps, and ensure proactive follow-up.

The Care Coordinator is tasked with ensuring continuity of care and effective communication among various healthcare settings.

Daily Duties Include:
  • Conducting outbound calls to assess patient needs and connect them with suitable resources.
  • Coordinating appointments with healthcare providers for patients.
  • Responding to incoming calls from patients and providers.
  • Following up with patients to confirm their needs are met and scheduling future check-ins.
  • Notifying patients of appointment details as necessary.
  • Collaborating with clinical resources to ensure a smooth care experience.
  • Assisting with resource applications and necessary paperwork.
  • Monitoring patient hospitalizations and coordinating with care team members.
  • Providing educational materials to patients and communicating with primary care physicians and specialists regarding new enrollments.
  • Working closely with all levels of the clinical team to manage care transitions and address any gaps.
Qualifications
Minimum of 2 years of relevant education, experience, or certification required. Current BLS or CPR Certification is necessary. Reliable transportation for commuting to patient locations is essential. In-person patient care may involve various physical activities. Preferred Qualifications:
Active Community Health Worker (CHW) Certification, customer service experience, and proficiency in MS Office applications are advantageous.About You
Excellent communication skills, both verbal and written, with a strong ability to handle confidential information discreetly. You demonstrate clear, respectful, and thoughtful communication.
