Role Responsibilities
The Care Coordinator will work closely with the care team to deliver ongoing support and communication to patients with chronic kidney disease (CKD) and End-Stage Renal Disease (ESRD). This position serves as a central point of contact, ensuring seamless coordination of resources throughout the care continuum.

Key Duties Include:
  • Conducting outbound calls to assess patients' clinical needs and connecting them with suitable resources.
  • Coordinating appointments with healthcare providers and following up on patient care.
  • Responding to inbound inquiries from patients and providers.
  • Ensuring patients are informed about their appointments and care plans.
  • Collaborating with clinical teams to maintain a smooth continuum of care.
  • Providing educational materials and communicating with primary care physicians and specialists regarding patient enrollments.

Qualifications
  • Minimum of 2 years of relevant education or experience.
  • Current BLS or CPR Certification.
  • Reliable transportation for patient visits.
  • Proficient in Microsoft Office Suite.
