Client Care Coordinator

Senior Helpers is expanding its Care Management division and is seeking a dedicated Client Care Coordinator. As a vital member of the Client Services team, you will play a key role in our mission to deliver compassionate care and enhance the quality of life for our clients and their families.
Key Responsibilities- Conduct assessments for new clients and perform ongoing evaluations.
- Create initial care plans utilizing the Life Profile system, ensuring accurate data entry into the Clear Care system, and adjust plans as necessary.
- Maintain regular communication with new clients, including caregiver introductions and follow-up visits within the first month of service.
- Compile care binders for each client, reviewing care plans with new caregivers.
- Deliver exceptional customer service, ensuring client and family satisfaction while addressing any concerns collaboratively.
- Conduct bi-monthly visits to all clients to evaluate service quality and client satisfaction.
- Collaborate with client teams to minimize hospitalization risks in accordance with the Life Profile system.
- Coordinate with the scheduling team to address caregiver changes and ensure client needs are met.
- Document all activities transparently in the office calendar within Clear Care.
- Assist in recruiting efforts to fill staffing gaps and promote service inquiries to achieve revenue targets.
- Represent Senior Helpers professionally at networking events and contribute positively to the company's marketing efforts.
- Participate in occasional after-hours responsibilities, including on-call duties.
- Strive for financial growth each month and explore opportunities for upselling services.
- Perform additional duties as assigned.
- High School diploma required; Bachelor's degree and relevant experience preferred.
- Experience in customer service, case management, or sales is essential; a background in healthcare, nursing, or social work is advantageous.
- Exceptional organizational skills and attention to detail are necessary, along with the ability to prioritize tasks in a dynamic environment.
- Strong verbal communication skills and follow-up abilities with clients and their families are required.
- Ability to build rapport with individuals from diverse backgrounds is essential.
- Creative problem-solving skills are a must.
- Ability to multitask while maintaining professionalism in a fast-paced setting.
- Proficiency in Microsoft Word, Excel, and Outlook is required; ability to learn new software quickly is necessary.
- Must be able to work independently as well as collaboratively within a team.
