Job Summary

The Clinical Coordinator RN will play a vital role in the coordination of clinical services, working closely with our clinical staff to provide high-quality care to patients and their families. This is an exciting opportunity for a motivated and experienced RN to take on a leadership role and contribute to the success of our team.

Key Responsibilities
  • Triaging Calls

    The Clinical Coordinator RN will be responsible for triaging calls related to patient care, including physician orders, staff issues, and other clinical issues. This will involve providing effective guidance to clinical staff and ensuring that patients receive timely and appropriate care.

  • Coordination of Clinical Services

    The Clinical Coordinator RN will work closely with our clinical staff to provide necessary services for patients and families in a timely manner. This will involve coordinating with other departments and teams to ensure seamless delivery of care.

  • Performance Evaluation and Development

    The Clinical Coordinator RN will be responsible for evaluating the performance of clinical staff and providing feedback and guidance to support their development and growth.

  • Compliance and Quality Assurance

    The Clinical Coordinator RN will ensure compliance with regulatory guidelines and Tufts Medicine Care at Home policies and procedures. This will involve monitoring and reporting any concerns or issues related to corporate compliance.

Requirements
  • Registered Nurse (RN)

    A minimum of three years of community health experience is required, or a combination of at least one year of acute medical/surgical nursing experience and two years of home health experience.

  • Leadership and Communication Skills

    The Clinical Coordinator RN must possess excellent leadership and communication skills, with the ability to work effectively with clinical staff, patients, and families.

  • Knowledge of Home Healthcare Regulations

    The Clinical Coordinator RN must have a working knowledge of home healthcare regulations, including Medicare Conditions of Participation and Agency Infection Control Plan.
