Job Summary

We are seeking a highly skilled and experienced Registered Nurse (RN) to join our team as a Clinical Care Coordinator. Key Responsibilities
  • Provide clinical support to the management team and coordinate the delivery of care to patients and families in their homes.
  • Triage calls related to patient care, including physician orders, staff issues, and other clinical issues.
  • Work closely with clinical staff to provide necessary services for patients and families in a timely manner, including the provision of direct patient care.
  • Communicate regularly with Clinical Managers and the Clinical Director on issues related to clinical practice or performance.
  • Demonstrate expertise and knowledge in areas such as current methods of care and treatment, policies, procedures, and protocols, infection control, reimbursement criteria, and regulations.
  • Assist with individual caseload reviews, audits, and orientation of clinical staff.
  • Evaluate performance of staff in relation to established standards and provide guidance and direction as needed.
  • Work collaboratively with all disciplines to facilitate productivity, cooperation, and efficient operations.
Requirements
  • Registered Nurse (RN) with a minimum of three years of community health experience or a combination of at least one year of acute medical/surgical nursing experience and two years of home health experience.
