Vinfen is seeking a skilled Nurse Team Coordinator to join our team. As a key member of our Adult Clinical Community Supports (ACCS) team, you will play a critical role in promoting the health and well-being of individuals with psychiatric, developmental, and behavioral disabilities.
Key Responsibilities:
- Coordinate access to and management of healthcare services for individuals, working in conjunction with care coordination entities and medical and behavioral health providers.
- Monitor and consult with team members regarding medical and psychiatric conditions, ensuring comprehensive care and support.
- Develop and implement health promotion strategies to address modifiable risk factors, promoting healthy lifestyles and behaviors.
- Oversee Medication Administration Program (MAP) compliance, ensuring accurate and timely medication administration.
- Collaborate with team members to develop and implement individualized care plans, addressing unique needs and goals.
Requirements:
- Registered Nurse (RN) licensure in Massachusetts required.
- Three to five years of supervised practical experience, including internships, preferred.
- Experience working with individuals with psychiatric disabilities and/or co-occurring disorders, preferred.
- Valid US driver's license and reliable vehicle for transporting individuals, required.
- Bilingual/bicultural applicants and those with lived experience of psychiatric conditions, preferred.
