About the Role:
We are seeking a highly skilled and experienced Clinical Coordinator RN to join our team at Tufts Medicine Care at Home. As a Clinical Coordinator RN, you will play a vital role in the coordination of clinical services, ensuring that patients receive high-quality care in the comfort of their own homes.
Key Responsibilities:
- Provide effective coordination of clinical services, consistent with the objectives of the agency.
- Work closely with clinical staff to provide necessary services for patients and families in a timely manner.
- Communicate regularly with Clinical Managers and the Clinical Director to address any issues related to clinical practice or performance.
- Assist with individual caseload reviews and audits as assigned by the manager or Clinical Director.
- Provide guidance to clinical staff on patient visit frequency, skill, and homebound status.
- Evaluate performance of clinical staff in relation to established standards.
- Assist in determining staff competency and provide direction as necessary.
- Work collaboratively with all disciplines to facilitate productivity, cooperation, and efficient operations.
- Triage calls from physicians and follow through on required actions.
- Provide documentation that is concise, accurate, and informative.
Requirements:
- Registered Nurse (RN) with three years of community health experience or a combination of at least one year of acute medical/surgical nursing experience and two years of home health experience.
