Join Our Team as a Licensed Practical Nurse in Home Hospice Care

Hospice & Community Care is a leading provider of quality care to patients and families in need. We are seeking a skilled Licensed Practical Nurse to join our team in a full-time home hospice care position.

About the Role

As a Licensed Practical Nurse in our home hospice care team, you will work collaboratively with our interdisciplinary team to provide personal care services and skilled nursing services to patients and caregivers. Your expertise will be essential in addressing urgent and ongoing needs, ensuring the highest level of care and comfort for our patients.

Responsibilities
  • Provide personal care services and skilled nursing services to patients and caregivers
  • Work collaboratively with the interdisciplinary team to address urgent and ongoing needs
  • Participate in patient assessments, care planning, and implementation
  • Communicate effectively with patients, families, and healthcare team members
  • Maintain accurate and up-to-date patient records
Requirements

To be successful in this role, you will need:

  • 1 year of LPN experience
  • Ability to work a minimum of 64 hours per two-week pay period
  • Must be vaccinated for influenza or be granted a religious/medical exemption
