About the LPN Care Manager Role

The LPN Care Manager is a critical position that requires travel throughout Licking County. This is a full-time role, 8:00am-4:30pm, with one weekend day rotation every other month.

Key Responsibilities
  • Provide superior care for patients in their homes or facilities, utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation.
  • Effectively interact with patients, families, and other interdisciplinary team members while maintaining standards of professional nursing.
Qualifications
  • Active LPN license in the State of Ohio without any board actions.
  • Two years of acute care nursing experience preferred.
  • CPR certification.
  • Computer skills sufficient to properly document services and care.
  • Ability to drive during daytime, nighttime, or inclement weather.
  • Valid driver's license with a safe driving record.
  • State minimum automobile insurance coverage.
