Metropolitan Jewish Health System is a qualified employer under the Federal Government's Paid Student Loan Forgiveness Program (PSLF).
Key Responsibilities:
As a Licensed Practical Nurse (LPN) under the supervision of an RN Case Manager, you will be responsible for visiting hospice patients in their homes or facilities. You will adhere to each patient's individualized care plan, providing direct care and documenting each visit using the Netsmart EMR system. It is essential to report any changes in the patient's condition to the RN Case Manager promptly.
This is a Full-Time, overnight position.
Qualifications:
- Graduation from an accredited Licensed Practical Nurse Program
- A minimum of one year of experience as an LPN in Med/Surg nursing and one year of experience in Hospice or Oncology Nursing
- Proficient computer skills are required
- A valid driver's license and a vehicle are necessary for patient visits, except in designated areas.
Compensation:
Minimum: USD $52,759.17/Yr.
Maximum: USD $65,948.96/Yr.
